Making a Patient Referral

 

Referrals to Providence Hospice and Home Care come from a variety of sources. Typically these include the person needing care, a family member, family physician, hospital personnel, friend, or clergy.

In order to qualify for home health care, the services must be intermittent, provide a skilled need, and in most cases the patient must be homebound. All services must be ordered by a physician.

In order to qualify for hospice care, the home services must be ordered by a physician and the patient must have a documented terminal diagnosis with an expected prognosis of 6 months or less, if the disease takes it's expected course.

Providence Hospice and Home Care does not provide private-duty or hourly services.
