Clinical Pharmacists

  • Develop a Plan of Care to meet each patient’s needs.
  • Schedule compounding of medications as well as assuring that patient supplies are accurate and available when needed.     
  • Monitor and trend lab values.     
  • Recommend therapy options and medication dosing.     
  • Monitor each patient’s response to therapy.     
  • Provide therapy-specific patient medication information.     
  • Screen patients for therapy duplication.     
  • Screen patients for drug-drug and food-drug interactions.     

Registered Nurses

  • Write an individualized Plan of Care for each patient.
  • Train patients and caregivers to manage infusion of therapies and to develop self-sufficiency. 
  • Visit patients for assessment, to monitor compliance, and to support independence.
  • Screen patients for nutritional risk.     
  • Insert and maintain peripheral, midline and PICC catheters.     

Registered Dietitians

  • Identify patients who are nutritionally at-risk.     
  • Conduct serial nutrition assessments on a regular basis to ensure patients are meeting their nutritional needs and goals of therapy.      
  • Consult with the pharmacist, nurse and physician for enteral, parenteral, and oral nutritional supplements and on-going nutritional management.      
  • Visit patients identified as needing nutrition counseling.

Reimbursement Specialists

  • Verify patient’s eligibility for home infusion benefits.     
  • Determine the coverage available for each patient.     
  • Inform patient of any financial obligations.

Patient Services Representatives

  • Phone patients to inventory their medications and supplies.     
  • Advise patients of delivery plans.     
  • Schedule and coordinate deliveries.     
  • Assist patients in rotating medications.
  • Coordinate with the care team to ensure adequate supplies are available in the home.

Warehouse Staff

  • Manage drug and supply inventory.     
  • Manage equipment inventory, including preventive maintenance.     
  • Coordinate deliveries.

Delivery Staff

  • Deliver medications and supplies in a timely manner.     
  • Remind patients of storage requirements for each delivery (i.e., refrigerate medications).
