Clarifying Your Needs and Goals
Coping with a serious illness can be difficult. Transitions can help you live the best you can with a life-limiting illness.

Transitions provides information, resources, and support services to adults and their families coping with a life-limiting illness.  If you or someone you know and love is seeking support and information, Transitions may be able to help.

It’s About Quality of Life
It’s hard to cope with the progression of a life limiting illness. Transitions helps you find the support and help you need to live life to its fullest. Isn’t that what life is about?

Transitions is the only care program of its kind in King County to support clients who have a serious illness with a prognosis of 12 months or less.

Transitions can help you clarify your goals and explore the scope of care and assistance you may need. Whether you are engaged in palliative (comfort) or curative medical treatment, Transitions can help you find aid and assistance that lets you have more energy to live your life.

It’s About Customized Support
Do you have questions about what to do next? Are you looking at lifestyle changes? Do you need help with errands or perhaps someone to listen, a friendly ear? Transitions can help.

Our Transitions Coordinator can provide a social work assessment of your needs and identify resources that could be helpful.  Our volunteers can provide respite for a caregiver, emotional support and companionship, even a fresh perspective. Our Clinical Nurse Liaison can assist you in addressing comfort issues with your doctor.

Who Is Eligible?
 Thanks to the generosity of the Providence Hospice of Seattle Foundation, Transitions is free to any adult (18 +) with a life-limiting illness who resides within King County. Clients may request a one time consult or services on an ongoing basis.

Clients currently receiving in-home services such as Respite, COPES, and Home Health are also eligible for Transitions. Our referrals are received from primary care physicians, nurses, social workers or others concerned about the well being of patients and families.
