Admissions at The Mount

 

For more information about availability or to apply for any of the programs listed below, please submit an online admissions inquiry form, send us an email or call us at (206) 938-6036. 

Hand in Hand Assisted Living
If you are interested in submitting an application to Providence Mount St. Vincent Hand in Hand Assisted Living:

  • All applicants interested in admission to the Hand in Hand Assisted Living program are encouraged to take a tour of the apartments as a first step. Please call (206) 938-6036 to schedule a tour.     
  • After the tour, an application will be given to the perspective resident. Upon receipt of the completed application, the admissions office will meet with the prospective resident to assess and discuss the application.      
  • The prospective resident (and their family if desired) will meet with the social worker for the apartments to further discuss the needs of the resident.

Skilled Nursing Neighborhoods
If you are interested in submitting an application to Providence Mount St. Vincent for Skilled Nursing:

  • An application form may be obtained through the admissions office.  Please call (206) 938-6036 or contact us via e-mail.  Admission to a Skilled Nursing Neighborhood requires a written application.
  • Upon receipt of the completed application, a clinical evaluation will be done and the admissions office will call to discuss availability. Please note that at times we are unable to admit at the time of application due to lack of available space. In that case, you name will be put on a wait list and you will be contacted as soon as a room becomes available.      
  • At the time of admission, we will request orders for admission and a summary of the resident's current condition from the resident's physician.
