Protecting the Privacy of Our Patients

We recommend you take the following safeguards to protect your personal information and yourself from fraud:

  • Do not give your social security number, credit card or bank account information to a caller.
  • If you have provided financial information to a caller, or if you suspect you may have been a victim of fraud, cancel your credit card(s) immediately or contact your financial institution to flag you account for suspicious activity.
  • Monitor your credit for possible identity theft by routinely reviewing your credit report. You can request a free credit report by going to the official, centralized Web site of the three major credit report companies: www.annualcreditreport.com.
  • If you have been a victim of fraud or identity theft, contact your local police department to file an Identity Theft Report, which can offer protection from debts resulting from an identity theft.

For more information regarding identity theft and what to do if you are a victim of it, visit the Federal Trade Commission's consumer protection Website at: www.ftc.gov/bcp/edu/microsites/idtheft.
