Programs and Services for Older Adults

 

Home Health Services

Our home health department provides skilled medical care for patients who are ready to leave the hospital or a skilled nursing facility but who still require skilled nursing or therapy. These services often serve as an alternative to hospitalization by providing assessment and skilled care wherever the patient lives.

Hospice

Expert, compassionate care is provided to adults and children with life-threatening illnesses when cure is no longer a realistic goal. The hospice team provides care where the patient lives, whether at home, in an adult foster home or an intermediate care facility. People with cancer, heart disease, AIDS and respiratory diseases often choose hospice care so that they can remain with their loved ones in a familiar, comfortable environment.

Home Infusion

Home Infusion brings intravenous (IV) therapy to patients outside of traditional hospital settings. In private homes, foster homes, residential care facilities or infusion suites, patients in Oregon and Southwest Washington can benefit from home infusion services. A nurse clinician and a pharmacist work closely with the referring physician to ensure appropriate, high-quality care.

Home Medical Equipment

In addition to home oxygen and medical equipment, Providence Home and Community Services provides home medical equipment, maintenance and services. Specially trained staff members are also available to evaluate the needs of each individual patient, either in the patient's home environment or in our office.

ElderPlace

ElderPlace is a community-based alternative to full-time nursing care. ElderPlace offers a complete package of health and social services to frail seniors, both in the home and at ElderPlace day centers.

Providence Senior Services in Medford

Services in Medford include flu clinics, Medicare assistance, a health resource lending library, free transportation to medical appointments for those with limited mobility, and a Lifeline personal emergency response system that helps seniors and disabled adults live independently.

Providence House

Recognizing that safe, affordable housing is essential to the overall health and independence of older adults, Providence Health & Services built, owns and operates Providence House. Here, low-income, frail seniors live with dignity and security, receiving the help they need with daily tasks.

Emilie House

Emilie House is a 40-unit apartment building operated by Providence Health & Services.  It has provided quality senior living accommodations for more than a decade. It has large studio and one-bedroom apartments. Each apartment has a private bath with shower / tub and a complete kitchen. Full carpeting and draperies are provided. The building features a laundry facility and a large community room for social gatherings.

ElderPlace is a community-based alternative to full-time nursing care. ElderPlace offers a complete package of health and social services to frail seniors, both in the home and at ElderPlace day centers.

Providence Benedictine's Care Community (Mt. Angel)

Since 1957, staff members of Providence Benedictine Nursing Center have devoted themselves to providing quality nursing care in a loving, supportive environment. Founded by the Benedictine Sisters, the Nursing Center has become a health care leader by developing innovative, nationally recognized care programs. We carefully designed our care community to meet our clients' changing physical, mental, emotional, social and spiritual needs.

Providence Benedictine provides several levels of services: Orchard House, a personalized living center; Garden Homes, adult foster care; the Nursing Center, featuring skilled nursing care and a Medicare-certified Short Stay Unit; Rehabilitation Services; and Home Health and In-Home Services. Our care community also includes the Child Development Center, the Institute for Long Term Care and the Providence Benedictine Foundation.

Rehabilitation

Providence Rehabilitation offers comprehensive therapy services provided by skilled, licensed rehabilitation professionals. Services include Physical Therapy, Occupational Therapy and Speech/Language Pathology.
