An individual is eligible for Providence ElderPlace if he/she is:
Age 55 or older
Living in OR willing to relocate to Multnomah county (with assistance from an ElderPlace counselor, if needed)
Medicaid eligible OR willing to private pay
Service eligible per Multnomah County Aging and Disability Services (an ElderPlace counselor will facilitate this determination if you do not know or have not been assessed)
Community placeable (meaning not in imminent need of nursing facility placement)
