Providence Home Health Palliative Care provides specialized care to patients with serious or life threatening illnesses.
Why Palliative Care?
Patients with chronic or life-threatening illness typically require a greater level of care management. The Providence Home Health Palliative Care program helps to manage these patients and intervene early to avoid a crisis.
Which patients are appropriate for Palliative Care?
- Patients who meet home bound status criteria. If you are uncertain about a patient’s home bound status, please go ahead an make the referral. We will make the determination as to home bound status.
- Patients with advanced or chronic illness who are still seeking treatment
- Patients requiring extensive case management
- Patients who have difficulty with pain and symptom control
- Patients who have frequent hospitalizations or emergency department visit
Palliative Care offers:
- Patient tailored nursing visits
- 24-hour telephone nursing support
- Recurring social work visits
- Patient updates
- Management of difficult symptoms
Including: pain, difficulty breathing, nausea, constipation, loss of appetite, fear, anxiety, confusion, depression and others.
- Addressing changing goals of care
Identifying patient wishes, assisting with complex advanced directives, helping with transition to hospice where appropriate.
- Coordination of Care throughout the illness
Providing guidance regarding available treatment choices, practical and emotional support for family and caregivers, financial guidance, individual and family counseling.
- Education
Helping to access community resources, support groups and referral services, patient education and providing information about a specific disease.

Payment of Services
Palliative care services are billed through a patient’s Home Health benefit.Most insurance carriers offer a Home Health Benefit, including Medicare.
